I hereby grant to Little Black Pearl (LBP), an Illinois corporation with its principal office at 1060 E. 47th St, Chicago, IL 60653 and its direct or indirect subsidiaries, successors and assigns (“Little Black Pearl and Carver 47 Cafe”) the absolute right and permission to copyright, use, reproduce and publish, for art, advertising, trade or any other lawful purpose whatsoever, any photographic portraits, pictures, videos or other likenesses of me, or in which I may be included, in whole or in part, by any and all means now or hereafter devised, in perpetuity, throughout the universe, without notice to me and without compensation.
LBP will not provide compensation for the use of participants’ image or likeness in any promotional or informational materials. LBP retains an irrevocable, perpetual, worldwide, royalty-free, non-exclusive right and license to use, publish, distribute, reproduce, copy and modify any intellectual property developed as part of programming. LBP owns or controls 100% of all rights, title and interest in and to the intellectual property, including but not limited to all copyrights, literary and artistic works, products, materials, information and computer software created or developed as a result of programming at LBP and its partner sites.
Throughout programming at LBP and partner sites, participants will create and collaborate on art pieces, including drawings, photographs, videos, music, sculptures, writings and performances. I give LBP permission to use, publish, reproduce, modify, distribute, and publicly perform or exhibit photographs, recordings, and any artwork I make or have made while participating in the program, in whole or in part, without restrictions or limitations, in any media now known or not yet invented and waive any claim to royalties or compensation.
I understand and agree that this is a complete release and discharge of all claims and rights I may have against LBP and that no action will be taken by or on behalf of me with respect to any such rights. I understand that this release shall be binding upon my heirs, executors, successors and assigns.
In order to ensure a safe and positive environment for LBP program patrons and participants, LBP has installed video surveillance cameras throughout the facility. Please be advised that this footage will be reviewed by LBP security staff from time to time.
